What Does It Mean to Have A.D.H.D.?

A diagnosis is not a simple brain test

APatients often ask psychiatrists if they have A.D.H.D., bipolar disorder or autism. Some people think a psychiatrist can find a clear problem inside the brain, like a doctor finding an infection. However, psychiatric diagnosis is more complicated. When a psychiatrist says that someone has A.D.H.D., it usually means that the person's behavior and experiences fit a pattern called A.D.H.D. The doctor makes this decision after listening to the patient, talking to people who know them and observing how they think and behave.

What diagnoses are for

BFor many years, mental health professionals have used a book called the D.S.M. It gives names and descriptions to different mental health conditions. These labels help doctors communicate, choose treatments and organize research. However, the labels can make people think that every mental disorder is completely separate from others. In reality, the boundaries between different conditions are not always clear.

What science shows

CScientists have studied genetics, brain scans and brain activity. They have found biological differences connected with mental health problems. However, these differences do not perfectly match the diagnostic labels in the D.S.M. For example, a brain scan cannot simply tell whether a person has depression, A.D.H.D. or autism.

Diagnoses are still useful

DThis does not mean psychiatric diagnoses are useless. They describe real patterns of problems and can help doctors choose possible treatments. However, doctors should use these labels as practical tools, not as perfect descriptions of a person.

Symptoms exist on a continuum

EMental health symptoms are not always simply present or absent. They often exist on a continuum. This means people can have different levels of a symptom. For example, some people may have very little difficulty concentrating, while others may have much more. Mental health traits can be similar to height: people are not simply tall or not tall. There are many levels in between.

Symptoms can affect each other

FMental health symptoms can also influence one another. For example, poor sleep may increase anxiety, and anxiety may make sleep more difficult. These symptoms can create a loop and make each other stronger. Mental health problems can also change over time. A person's symptoms may fit one diagnosis at one stage of life and another diagnosis later.

Life and personality also matter

GA diagnosis does not always explain the relationship between a person and their environment. Sometimes a person's natural traits do not fit well with what school, work or family life requires. Some uncomfortable feelings may also have a useful purpose. Anxiety, for example, can sometimes warn us about danger. Personality is also important because different people can experience and respond to mental health problems in different ways.

One person may receive different diagnoses

HThe writer often sees patients who have received several diagnoses during their lives. One doctor may diagnose depression and anxiety, another may diagnose bipolar disorder, and another may diagnose post-traumatic stress disorder. Patients often ask, "Which disorder do I really have?" The writer's answer is complicated. Each diagnosis may describe part of the person's experience, but no single label explains everything.

Mental health is complex

IMental health problems are not usually caused by one simple thing that a person either has or does not have. They are patterns shaped by biology, personality, experiences and life circumstances. This explanation is more complicated than saying someone has a simple "chemical imbalance" or "brain disease." However, the writer believes it is closer to the truth.
